Position description
The Betty Irene Moore School of Nursing at UC Davis seeks innovative and highly motivated doctorally prepared Adult Gerontological Primary Care Nurse Practitioners (AGPCNPs) or Family Nurse Practitioners (FNPs) to join the faculty. The AGPCNPs/FNPs will serve in a unique role providing clinical care to older adults, provide clinical teaching activities, and participate in clinical research. While all Nurse Practitioners are welcome and encouraged to apply, the school is particularly interested in NPs with a keen interest in Dementia Care, Caregiving, and improving experience of care and optimizing outcomes for patients with dementia.
These NPs will serve in a unique role that includes providing clinical care to older adults, teaching students, precepting APP fellows, collaborating in research activities, and leading the CMS Guide Program https://www.cms.gov/priorities/innovation/innovation-models/guide in collaboration with the geriatric medicine team. They will have recognized expertise in the care of older adults, particularly the holistic assessment and management of older adults with dementia, chronic co-morbid diseases, and/or frailty. This position will split their time between clinical responsibilities, teaching, service, and research or scholarly activities.
This position will be responsible for providing patient care in the healthy aging clinic collaborating with geriatricians, advanced practice providers, and other clinicians and staff. This position, working in a team-based care environment, will also serve as a clinical resource in providing clinical care for older adults including co-developing multifaceted treatment plans, providing patient education, and participating in and contributing to clinical care huddles. In the clinical practice environment, they will work under the direction of the Healthy Aging Clinic leadership in a team-based care environment, precept students in the clinic and report to the Associate Dean for Practice for practice related issues. This position may also have teaching responsibilities as assigned in the SON programs (DNP-FNP, PhD, MEPN, and PA programs), reporting to the Associate Dean for Academics. The NPs will also be responsible for ultimately developing a program of research in collaboration with the Healthy Aging Clinic leadership and other researchers with similar research interests.
Part-time and full-time, non-tenured positions at the Assistant and Associate rank in the Health Sciences Clinical Professor (HSCP) Series or full-time Clinical X Series are available at UC Davis Health, which is located on a modern, 140-acre campus in Sacramento, California. The Sacramento campus is home to the School of Nursing, School of Medicine, UC Davis Medical Center (including a level-I trauma center and burn unit, and a full range of outpatient services), a fully accredited Children’s Hospital, and an NCI-designated Comprehensive Cancer Center.
The budgeted salary range for this position is $140,000 to $154,000. Assistant or Associate salaries are commensurate with experience and qualifications. This position includes membership in the Health Sciences Compensation Plan.
CLINICAL RESPONSIBLITIES:
Successful candidates will provide clinical care to geriatric patients, caregivers, and precept students. Duties will include:
Provides independent advanced level nurse practitioner clinical care to older adult patients.
• Works collaboratively within the team.
• Facilitate caregiver’s capabilities to provide safe care to their older adult relative or friend working with the AGCNS and other providers.
o Ensure that caregivers understand medication management, wound care, incontinence care, behavior management and other identified patient challenges as indicated; may refer to other clinic providers.
o Care coordination to help caregivers navigate the health system.
o Advance Care Planning
• Participate as a member of the team in clinic wide initiatives:
• Dementia care
• Behavioral Health Integration
• Complex Care and Medical Team conferences
• Transitions Care Management
• Chronic Care Management
• Mobility
• Wellness
• Functions as an advanced practice nursing leader in collaboration with medical director and other leaders of the Healthy Aging Initiative.
• Provide direct care to older adults as indicated in alignment with the team-based care collaborative approach.
• Collaborates with leadership and clinic staff to develop systems and methods to assist/improve resource utilization and additional services.
• Collaborates with leadership to encourage practice environments that support shared decision-making, employee engagement, and interdisciplinary coordination of care.
• As needed, serve as a clinical expert resource for staff and students and provide geriatric screening examinations to older adults. These exams may include examples such as but not limited to:
o Mini Mental Status Exam or MOCA or Mini-Cog
o Unintentional Weight Loss Screening Tool
o Functional Screening (Lawton IADL, GAF)
o Geriatric Depression Scale
o Nutrition assessments
o Pain assessments
o Precept advanced practice students and fellows
o Documents care comprehensively in the electronic health record.
RESEARCH AND SCHOLARSHIP RESPONSIBILITIES:
• Develops a program of clinical research:
o In collaboration with the Family Caregiving Institute, Associated Medical Departments, SON Associate Deans for Research and Practice, develops a program of funded research focusing on family caregivers and related topics of interest in the care of older adults OR
o In collaboration with other members of the healthy aging community and SON, develops a program of funded research.
• Participates in Family Caregiving Institute or other UCDH or SON research consortium activities.
• Disseminates research findings to national audiences.
TEACHING, MENTORING & SERIVCE RESPONSIBILITES:
• Teach and mentor students in areas of expertise as assigned by the Associate Dean for Academics.
• Provide service to the School of Nursing, Health System, and University through committee participation as assigned.
• Provide service to the community and profession.
CRITERIA:
A candidate for appointment in the HSCP Series is evaluated by the following criteria, which is appropriately weighted according to the primary emphasis on clinical and clinically relevant teaching and patient care services and according to the needs of the school and the individual’s responsibilities in the specific discipline. The four criteria are:

Qualifications
Graduation from an accredited school of nursing with a BSN or AND plus doctoral degree (e.g. PhD, DNP, EdD) is required. The candidate must meet the following qualifications:
• Doctoral degree in nursing or related field with AGPCNP or FNP certification and or/equivalent combination of experience/training.
• Experience in geriatric nursing care field.
• Licensed as an Adult Gerontological Primary Care Nurse Practitioner or a Family Nurse Practitioner with experience caring for older adults.
• National Board Certification
• Possess a valid and unrestricted California RN and NP license.
• Possess a DEA license or Completion of the Schedule II opiate prescribing course in preparation for DEA.
• Experience in geriatrics and working in a team-based care environment.
• Current Basic Life Support (BLS) certification.
• Ability to work in a clinical setting up to 3 days a week.
• Demonstrated leadership knowledge and skills to establish and maintain a culture of patient safety.
• Ability to work well in a team-based environment.
• Ability to motivate and teach learners.
• Ability to integrate the understanding of the Health System’s and School of Nursing’s mission, vision, core values, commitments and projects within a service oriented health care environment.
• Ability to incorporate knowledge of Joint Commission and Title 22 (and other regulatory bodies) rules and regulations to ensure that clinic programs and operations are in compliance.
• Knowledge of nursing standards and competencies in ambulatory care, particularly in the care of older adults.
